On March 10, 2017, Jordan Valley Medical Center West Valley Campus became aware of a breach of patient PHI that occurred when filing proof of claim with the United States Bankruptcy Court for the District of Utah. This breach may have included the patient’s (a) social security number, (b) date of birth, and/or (c) medical account number. A description of the healthcare services provided to the patient was also included in the proof of claim in the form of an itemized statement.
Once we learned of the incident, we promptly investigated and took immediate steps to remediate the issue.
